How to submit a death claim?

You are required to notify us of a death verbally or in writing.

The following documents are required to process the claim:

  • Death Certificate
  • Birth Certificate
  • Statement of Death (form provided by Assuria)
  • Police Report (if necessary)
  • Letter of Administration if the Policyholder’s beneficiary is an estate.
  • Beneficiaries Identification/Birth Certificate

All death claims are authenticated by our Medical Advisor.
