How to submit a death claim?
You are required to notify us of a death verbally or in writing.
The following documents are required to process the claim:
- Death Certificate
- Birth Certificate
- Statement of Death (form provided by Assuria)
- Police Report (if necessary)
- Letter of Administration if the Policyholder’s beneficiary is an estate.
- Beneficiaries Identification/Birth Certificate
All death claims are authenticated by our Medical Advisor.
